The first seven forms above forms are required for the first day of attendance
The first seven childcare forms listed above are required for the first day of attendance. In addition, the following enrollment forms must also be submitted on that day:
•Copy of Birth Certificate or Passport for identification
•Essay “One Day with My Child”
The remaining forms are required as needed.
